1) Private dining event of 30 people or more
must be booked and non-refundable security deposit received
by our private dining department by December 1, 2002, to
be eligible to enter sweepstakes.
2) Must be 18 years of age or older, a legal
U.S. resident, and a recipient of an official sweepstakes
entry form to enter. To obtain an official sweepstakes entry
form, please visit or website www.signatureroom.com or contact
Estela Huizar at (312) 280-0455.
3) To enter, complete and mail legible entry
form to The Signature Room at the 95th®, 875 North Michigan
Avenue, Chicago, IL 60611 by December 1, 2002. Entry form
must be completed in full including any survey questions
to be valid.
4) Prizes: first-class airline tickets for
2 people for a trip around the world (up to 10 stops) aboard
United Airlines and Star Alliance Partners. One drawing
and one prize per each of our two locations will be held
and awarded. Approximate retail value is $20,540.00 ($10,270.00
per prize.) Trip must be taken by December 31, 2003. Travel
Arrangements are at Sponsor’s approval and availability.
5) Odds of winning depend on total number
of valid entries received. One entry per booked function.
6) Winners will be selected randomly by United
Airlines and Star Alliance Executives and announced at a
cocktail reception to take place at The Signature Room at the 95th®, 875 North Michigan Avenue, Chicago, IL 60611,
on December 5, 2002. Official invitations will be sent via
U.S. mail to all eligible participants. Winners need not
be present. Winners will be notified by mail no later than
December 31, 2002.
7) Substitute winner will be selected if winning
entry does not comply with these Sweepstakes Rules.
8) Winners and travel companion must execute
affidavit of eligibility and release Sponsor, affiliated
businesses, and agents, from any liability claims or actions
for any kind of injury, death or other loss ("Death
or Damage") to persons or property sustained in connection
with receipt, enjoyment, ownership or use of a prize, including
Death or Damage incurred in travel to or from a prize trip.
Sponsors disclaim any responsibility or liability for Death
or Damage relating to sweepstakes and prizes.
9) Winners are responsible for transportation
to and from destination and arrival points, hotel accommodations,
meals, and for any other expenses that may be incurred on
the trip.
10) Promotion sponsorship by United Airlines
and Star Alliance Partners, 1200 East Algonquin Road, Arlington
Heights, IL 60005.
11) Employees or immediate family members
of The Signature Room® and its affiliates, United Airlines
and Star Alliance Partners or any other "Sponsors"
that are cooperating with the "The Signature Room® wants
to give YOU the world!" promotion, are not eligible
to enter. Proof of eligibility may be required.
12) Void where prohibited by law. Subject
to all, federal, state and local laws and regulations. All
taxes are winner’s responsibility
13) No purchase necessary. To enter without
booking an event, complete step 3 above, and submit with
your entry a 50 word or less explanation of why you love
The Signature Room at the 95th® or The Signature Room at Seven Bridges®. One entry per person.
14) By entering, winners and their travel
companions consent to use of their name and likeness for
promotional purposes without additional consideration, unless
prohibited by law.
